Pre-Massage Assessment

When making your massage appointment, please be prepared to answer a few questions related to your health and medical history. These questions will help me to determine how best to meet your physical needs.

Below is a sampling of some of the questions you may be asked during this interview:

1) Have you ever had a massage before?
2) What is the reason for your visit?
3) What is your occupation?
4) What (in general terms) are your work responsibilities?
5) How would you describe your current level of stress?
6) How is your health in general terms?
7) Have you experienced any changes in your health in the past year? If yes, please explain.
8) Are you currently being treated by a physician or taking any medication?
